At Art of Palliative Care, we support individuals with complex medical and social needs through a coordinated, whole-person approach. Our services integrate Enhanced Care Management (ECM), Justice-Involved (JI) program support, and palliative care to ensure individuals receive consistent, accessible, and patient-centered care.
We understand that navigating healthcare systems, managing chronic conditions, and transitioning between care settings can be overwhelming. Our role is to bridge these gaps by providing structured coordination, clinical support, and access to essential services.

The Art of Palliative Care (APC) und die San Jose University (SJU) freuen sich, eine gemeinsame Initiative bekannt zu geben, die Praktikumsmöglichkeiten für Studierende des Sozialarbeitsprogramms der SJU bietet. Ziel dieser Partnerschaft ist es, den Studierenden wertvolle praktische Erfahrungen im Bereich der Palliativpflege zu vermitteln und gleichzeitig die Mission von APC zu unterstützen.

Art of Palliative Care ist ein spezialisiertes medizinisches Team, das sich auf die Betreuung von Patienten und ihren Familien konzentriert, die von den Symptomen und Stressfaktoren einer schweren Krankheit betroffen sind – unabhängig von der Diagnose. Wir arbeiten mit Ihrem Arzt zusammen, um Ihnen bei der Schmerz- und Symptomkontrolle, emotionaler und spiritueller Unterstützung, Kommunikation und Koordinierung der Pflege zu helfen, um Ihren Gesundheitszustand effektiv zu behandeln. Die Leistungen von Art of Palliative sind nicht von der Prognose abhängig und können gleichzeitig mit der kurativen Behandlung erbracht werden.
