  • Who Will Visit Me from the Art of Palliative Care Team?
    Our multidisciplinary team includes compassionate healthcare professionals dedicated to supporting you. Here’s an overview of who may visit you: Nurse Case Manager Our experienced nurse case managers specialize in palliative care for individuals with advanced illnesses. They work closely with your physicians to help you meet your treatment goals. By coordinating with our palliative physicians and nurse practitioners, your nurse case manager ensures your pain and symptoms are effectively managed. Palliative Care Physician or Nurse Practitioner A board-certified palliative physician or nurse practitioner will help address pain and other symptoms, using evidence-based care plans designed to relieve suffering. They collaborate with your other healthcare providers to enhance symptom relief and help you stay comfortable. Medical Social Worker Our medical social workers are here to support you and your family with resources and services you may need. They offer assistance with practical needs like food, housing, transportation, and supplies, as well as emotional and educational support throughout your journey. Chaplain Our non-denominational chaplain is available to provide spiritual support for you and your family, respecting all beliefs and practices.

Your care team will include: You and Your Family At the heart of our care model, you and your family are active members of the care team, guiding decisions based on your unique needs and values. Primary Care Physician Your primary care physician plays a crucial role, continuing to oversee your general healthcare needs. Palliative Physician or Nurse Practitioner Our specialist provider focuses on managing symptoms and relieving discomfort, working in harmony with your current treatments. Nurse Case Manager A skilled nurse coordinates your palliative care, ensuring consistent support and symptom management. Social Worker Our social worker assists with resources, emotional guidance, and any challenges you may encounter. Care Coordinator Our coordinators help manage appointments, ensure smooth communication between team members, and address your logistical needs.
